Do to extensive requests for waivers, I am adding printable HTML versions to the website at these locations.
ADULT
MINOR
If you are an ADULT (18+), I will require proof of age when you bring the waiver to me. Any of the following will suffice: State ID, Drivers Liscense, School ID (with both birthday and picture).
If you are a MINOR (16-17), I will require proof of age and proof that your guardian did indeed sign the paperwork (and not your best friend). Proof will consist of your guardian coming to the field, your guardian calling me personally or someone I trust vouching for you.
